Newborn Home Visit

The Home Visit Includes:

Review of the baby's and the mother's health by a registered nurse
Sharing of parenting and resource information
Answering Questions about newborn care
Offering helpful tips about feeding, baby growth, and creating a healthy and safe environment for the baby
Linking the family to community resources via information and referral
Promoting Literacy

The Help Me Grow Service Coordinator Will:

Provide in-home parent education and support
Offer developmental screenings/evaluation for your child at no cost
Make sure your baby reaches important milestones
Help keep immunizations up-to-date
Help you locate resources to meet your family's needs
Help you develop and reach family goals
Ensure that an Individualized Family Service Plan (IFSP) is developed based on your child's unique needs and your family's goals and priorities
Determine if your child is
eligible for early Intervention
Help you explore options and select services
Offer information about the Individuals With Disabilities Education Act (IDEA) and Parent's Rights in Early Intervention
Link you to other community resources and information
