Newborn Home Visit
The Home Visit Includes:
- Review of the baby's and the mother's health by a registered
nurse
- Sharing of parenting and resource information
- Answering Questions about newborn care
- Offering helpful tips about feeding, baby growth, and
creating a healthy and safe environment for the baby
- Linking the family to community resources via information
and referral
- Promoting Literacy
Help Me Grow Ongoing Services
The Help Me Grow Service Coordinator Will:
- Provide in-home parent education and support
- Offer developmental screenings/evaluation for your child at
no cost
- Make sure your baby reaches important milestones
- Help keep immunizations up-to-date
- Help you locate resources to meet your family's needs
- Help you develop and reach family goals
- Ensure that an Individualized Family Service Plan (IFSP) is
developed based on your child's unique needs and your family's
goals and priorities
- Determine if your child is
eligible for early Intervention
- Help you explore options and select services
- Offer information about the Individuals With Disabilities
Education Act (IDEA) and Parent's Rights in Early Intervention
- Link you to other community resources and information
